Credentialing


Credentialing you with insurances that we work with at NLBH is the prerequisite step to begin working with clients.  There are several steps involved with this process, and they need to be completed in the order outlined below!


Step One


Sign up for a National Provider Identifier (NPI)  {the NPI is a unique number associated to you as a professional that will be used in your credentialing}


First check to see if you already have one: https://npiregistry.cms.hhs.gov/ .  If you do not have one, you will need to sign up as an individual provider here: https://nppes.cms.hhs.gov/#/  


Taxonomy Code:  You will need to choose a taxonomy code for your provider type.  For your licensure type, choose one below:

Note: Although there are other choices for each provider type, the ones above are the ones that are able to work with Medicaid for the three provider types.


Step Two


Complete a CAQH Profile {the CAQH profile is like having your resume on file as a reference for credentialing}


Your profile will need to be completed in order to turn in all other credentialing application.  Go to the CAQH website and sign up for an account.  If you think you might already have one (sometimes they are created by other organizations you work with without your knowledge) give them a call at 1-888-599-1771 to see if that is the case.  


Be sure to make sure your CAQH status is SUBMITTED ( you can find this in the top right hand area when you log into your account).  You will also need to go to the documents tab to make sure that all requested documents are uploaded and attest each time you make changes to your profile.


Your CAQH profile will need to be updated quarterly.  They will email you at the email on file, and you will need to re-attest each time, as well as look at additional documents that are need to 


Step Three


Register and complete your profile with NC Tracks  {All Medicaid providers are required to individually register with NC Tracks} 

Step 1:  Register for an NCID at their website which will provide you with a username and password to then start the NC Tracks application.

Step 2:  Complete an NC Tracks profile.  Visit this link NC Tracks application to start the process.  All information  will pertain to you as an individual, as NC Medicaid requires that you register independently and then link yourself to agencies, including New Leaf. The only exception is that you may use New Leaf's address is the practice address.  


Several key selections to make in the application process:   

1. You are a rendering/attending provider only. 

2. You will report your income to a SSN. 

3. You will need the following information to associate yourself NLBH in the organization affiliation section:  Tax ID is 32-0204218 and our NPI is 1508042003.  Please download the linked instruction manual for a step by step guide in completing your application.  You can also call them directly if you have any questions, and I encourage you to do that as well.  


Before submitting your application, we would like to review it to ensure everything is filled out correctly.  This will help cut down on processing time and get you working with clients faster.  Send the following information to dmuserallo@nlbh.org to review your NC Tracks application.

Additional Credentialing


Additional credentialing paperwork will be needed throughout the onboarding process.  NLBH staff will be responsible for the submission of these additional forms and may need your assistance with submission.  Staff will reach out directly to you to coordinate any additional credentialing paperwork needs.
